US & Mexico: US reports first deaths related to ongoing parasite outbreak linked to Mexico.
Michigan’s health department confirmed on 3 August that two individuals had died from an ongoing cyclosporiasis outbreak. The confirmed deaths mark the first known fatalities related to the US’s largest recorded outbreak of the parasitic intestinal disease. However, both individuals reportedly suffered from significant underlying health conditions that may have been aggravated by cyclosporiasis and the associated dehydration.
The US has now recorded more than 20,000 confirmed or suspected cyclosporiasis cases during the wider 2026 outbreak. Michigan accounts for 11,000 cases and almost 200 hospitalisations, although health officials have not identified a single source for all the infections in the state. However, lettuce and salad greens remain the principal suspected sources, with a separate cluster of almost 2,000 cases across nine states directly linked to iceberg lettuce supplied by farms in central Mexico.
Solace Global Assessment:
Cyclospora is a microscopic parasite primarily transmitted through food or water contaminated with faecal matter. Common symptoms include prolonged watery diarrhoea, abdominal cramps, nausea, fatigue, appetite loss, weight loss, and dehydration. The disease rarely spreads between people, is treatable, and rarely results in fatalities, with the deaths in Michigan almost certainly reflective of the vulnerability of people with serious underlying health conditions, combined with the scale of the outbreak.
However, the current scale of the outbreak and absence of a single confirmed source for the infections likely indicate the disease will continue to spread and result in further hospitalisations and potentially fatalities. On 31 July, the UK Health Security Agency (UKHSA) published new data indicating a sharp rise in cyclospora infections in the UK. Travel information was available for 51 out of the 67 cases, and of these, 48 cases reported travel to Mexico, particularly to the Riviera Maya and Cancún regions. The report indicated that those infected had stayed at a range of different hotels in the area and consumed a variety of food and drink as part of all-inclusive holiday packages.
The UK report likely illustrates the challenges of identifying and containing Cyclospora outbreaks when contaminated produce is distributed across multiple hotels, restaurants, and supply chains, making it harder to isolate or recall the affected products. With August being one of the busiest periods for travel from both the US and UK to Mexico, and the source of the outbreak remaining unidentified and potentially highly distributed, further travel-associated cases are likely in the short term, with many cases likely not being reported as symptoms may be mistaken for other gastrointestinal illnesses or resolve without a formal diagnosis.
